In our instance of Moodle, we use the Questionnaire plug-in instead of the feedback activity. This is a flexible tool that allows you to ask a lot of different question types, including branching questions that follow specific responses. These instructions describe how to use this activity.
A couple of nice features about this tool include being able to award points to learners who submit responses and setting up templates that teachers can use as a starting point for their own questionnaires. In our site, we have a template for a questionnaire that students complete to let us know how they plan to have an exam proctored. All the teacher has to do is select the template from a list and they are good to go!
